Please use the following form to let us know what you would like to do (select all that apply).  Once you submit the form, please check your email for a confirmation form.  Should you not see it, please check your spam folder.  If you do not confirm via email, your request cannot be completed.

Please note, requests to delete personal information may only be requested 30 or more days following a transaction. Within this 30 day period, we will need to maintain personal information to ensure orders are fully fulfilled and any order charges have successfully cleared payment processing.  If you should request your information be deleted, you relinquish your right for a refund, replacement or return as we will no longer have your information on record for the purchase.
